Gu Ben Shen Qi Tang comes from modification of "Shen Qi Tang" in "Golden Cabinet", and the Characteristics is tonifying kidney qi without pathogen retention, benefiting damp without hurting vital qi. However, the prescription falls short of tonifying yang when treat the edema due to deficiency of spleen and stomach yang, therefore, apply Chao Bai Zhu, Ba Ji Tian, Sheng Huang Qi, Lu Dang Shen, Che Qian Zi and increase the dose of Gui Zi and Fu Zi (15g for each), and name it as Gu Ben Shen Qi Tang. The patients who suffer from this disease due to deficiency of spleen and kidney yang have pale appearances, cold limbs, sore of lumber, fatigue, edema of whole body, especially the lower limbs, less eating while abdominal distention;  loose stools, flabby and pale tongue, or tooth mark, white and moist tongue coating, deep and retard pulse, or weak pulse.

The clinical syndromes of the rebound chronic nephritis patients who are treated by hormone are slight or moderate edema, sore lumber, fatigue, dizzy, tinnitus, feverish palms and soles, night sweating and fever,etc., the syndromes of deficiency of liver and kidney. The treatment should tonify liver and kidney, benefit damp and water by Liu Wei Di Huang Tang. In the recovery stage, when the patients get deficiency of qi and yin, apply Gou Qi Zi, Nu Zhen Zi, Han Lian Cao, Wu Wei Zi in Gu Ben Shen Qi Tang to benefit qi and yin to relieve the fatigue status, and treat low proteins syndromes. For the patients who are server of edema on the whole body, especially on the lower limbs, subtract Shan Yu, Di Huang, apply Di Fu Zi, and increase the dose of Che Qian Zi in Gu Ben Shen Qi Tang; it is good for benefiting water without hurting vital qi. Apply Mu Li San if the edema is in the vulva. For the treatment of albuminuria, the albuminuria exists in the whole course of nephritis, especially when the edema relieve or wear off, the patients still has albuminuria for a long term. The different treatments should be taken.

For the deficiency of spleen and kidney, unconsolidation of essential substance, apply Qian Shi 50g and Sheng Long Gu and Mu Li 50g (for each). For the deficiency of kidney, apply Fu Peng Zi, Jing Ying Zi, Wu Wei zi and Gou Qi Zi in Gu Ben Shen Qi Tang to tonify kidney; for deficiency of yin, modify Bu Zhong Yi Qi Tang and Gu Ben Shen Qi Tang to raise yang and consolidate kidney; For the retention of internal damp and heat, downward flow of essence, subtract Shan Yu, Shu Di Huang, Gui Zhi, Fu Zi and apply Han Fang Ji, Bi Xie, Hua Shi, Shi Wei, Huang Bai and Fu Ling in Gu Ben Shen Qi Tang to clear heat and benefit damp. The patients usually have not only the yin deficiency syndromes, but also damp heat syndromes due to the hormone application. It seems like oil mixed with powder, hard to be separated, and the routine test results of albuminuria are between ++--+++. It is therefore difficult to treat: it will hurt yin if benefit the damp and heat; it will disturb the damp and heat if nourish the yin fluid. Subtract Gui Zhi, Fu Zi, Ba Ji Tian, Huang Qi, Lu Dang Shen and apply Fang Ji, Bi Xie, Di Fu Zi and Fu Ling. It is common to see that the patients have stagnation caused by deficiency due to persistent disease intruding into collateral. The main complains are: squamous and dry skin, dark of both eyes, stinging sensation on lumbar, dark and pale tongue coating with petechiae which shows blood stasis; the albuminuria of these patients is an indolent. On the basis of differentiation, apply Yi Mu Cao which benefit water and activate blood, Ze Lan, with Chao Tao Ren, Hong Hua, Chi Shao, Dan Shen and Niu Xi,etc, especially prefer to apply Da Huang Dong Chong Wan. The Da Huang Dong Chong Wan is the key herbs to treat deficiency and eliminate blood stasis, therefore, combining with Gu Ben Shen Qi Wan to treat nephropathy with blood stasis is best.
