Internal Medicine

According to the close relationship between zang-fu organs, we treated 18 patients of gastroptosia by acupuncture on back-shu points and received satisfactory effect. Our introductions are as follows.
1 General Data:
18 cases in this group, 2 cases are in hospital and the others are outpatients. There are 7 males and 11 females. The eldest is 62 years old and the yongest is 19 years old. The shortest diseases course is half a year and the longest is 20 years.
2 Diagnostic Criteria:
1.Typical clinical syndromes: poor appetite, abdominal distention after meal, bearing down sense, gastralgia, belching, fatigue, etc.. 2. According to the barium meal examination and X-ray examination, arcus minor ventriculi is below the connecting line of cristae iliaca, inferior pole of stomach is more than 6 cm below the connecting line of cristae iliaca.
3 Treatment
The needle: NO.28~NO.30 acupuncture needles. The acupoints: Ganshu (BL18), Danshu (BL19), Pishu (BL20), Weishu (BL21). Time of therapy: 3 hours after meal is the best. Needling methods: The patients should be prone position; after skin sterilizing, the doctor punctures to the intervertebral foramen and the opening at nervi spinales transversely with acupuncture needles. Stringently control the depth and the direction to avoid medical accidents. 1~1.5 cun needles are most commonly used. Twirl the needles more than 20 times, pause for 1 minute, then withdraw the needles after repeating twirling. Suggest the patients to raise the legs straightly with 15~25°(put the legs on the head of bed or quilt) to take a 30-minute break after withdrawing. Take in some digestible food during the treatment courses. They should raise legs for 30 minutes to take a break after pucture or during the treatment periods.
4 Results
  (A) curative effect criteria: the patient is considered as cured if the clinical syndromes disappear, and the location of stomach returns to normal according to the review of barium meal examination; excellent if the clinical syndromes are improved obviously, and the inferior pole of stomach turns back 3~6 cm according to the barium meal examination; improved if the clinical syndromes are improved and the inferior pole of stomach turns back 1~2 cm according to the barium meal examination; inefficient if the inferior pole of stomach turns back 1 cm or less than 1 cm according to the barium meal examination no matter the clinical syndromes disappear or not. (B) Curative effect: all the patients in this group reviewed after one course. 6 cases are cured; 3 cases are excellent; 6 cases are improved and 3 cases are inefficient (it is considered as inefficient if the treatment continues for 2~3 disease courses).
5 Experience
There are lots of reports about gastroptosia. Generally speaking, the treatment time is long and most of the patients can not stick on. Among the 18 patients, 16 patients have tender spots on BL20 and BL21, and it disappears as the disease progresses. The facts indicate that: the back shu points not only react the pathological changes of zang-fu organs, but also is the specific acupoints for treatment. The patients have more stronger upwards girdle sensation of stomach when the gastroptosia is treated by acupuncture with back shu points. This treatment has good curative effect and short course.
The acupuncture treatment is good for the return of stomach when stomach emptying. The raising-legs method is still need more further observation on stomach returning because it comes from folk.
