In the 1014 cases of facial paralysis, 600 cases are mainly treated by acupuncture. 414 cases are divided into both needling and moxa group, needling group, reed moxa group, ginger moxibustion group, laser group and acupoints injection group at random. The 6 groups are put in the charge of persons specially appointed and do the observation on them respectively.
Treatment methods:
1. Both needling and moxa group: The main prescriptions: Xiaguan (ST7), Jiache (ST6), Dicang (ST4), Extra Taiyang, Quanliao (SI18), Sibai (ST2), UB2, Fengchi (GB20), and Hegu (LI4) Yuan-Source Point; the secondary points are Yingxiang (LI20), Yangbai (GB14), Shuigou (GV26), Chengjiang (CV24), Extra Qianzheng, LV3, SP 6 and Jianshi (PC5) Jing-River Point. Choose 5-7 points for acupuncture each time, and choose different points alternately. Apply with even method or needle warming method. After receiving qi, fix one cun moxa on the needle body which locate at ST7 and SI 18, and burn them for moxibustion. 1-2 cones for each treatment; it is better that the skin near the moxibustion turns pink.
The treatment time and courses: For early stage cases, do acupuncture once a day and last for 6-7 days; when the disease getting better, do acupuncture the each other day, 12 times are one course( following are same). Take 3-5 days break during every course.
2. Needling group: Apply even method or needle warming method. Retain needles for 20-30 minutes after receiving qi. Treat the patients everyday or on alternate days, 5-7 needles for each time; if necessary, apply point penetration method.
3.Reed moxa group: Preparation of reed device: At present, there are 2 kinds devices for usage. One is named as one-segment reed moxa device. The diameter of the reed caliber is 0.4-0.6 cm, and the length is 5-6 cm. Make one end of the reed into duck-like bill shape, and close another end with plaster for moxibustion in the antrum auris. Another kind is called two-segment reed moxa device: The caliber which hold the moxa is thick; the diameter is 0.8-1 cm, and make it into 4 cm long duck-like bill shape. The caliber which inserts into the antrum auris is thin; the diameter is 0.6-0.8 cm and the length is 3 cm. Put one side in the caliber which hold the moxa and connect each other, then the moxa device forms, so it is named as two-segment reed moxa device. The end which inserts into the antrum auris is fixed by plaster for moxibustion.
Moxa method: Put a cluster of moxa about the size of a half peanut in the duck-like bill place, burn it with joss-stick, and put the closed end into the antrum auris. It would be better to make the ear warm by moxa. 3-9 cones for each moxibustion.
Remarks: Avoid burning clothes or skin when performing moxibustion. It is better to looked at yourself in the mirror to see the burning condition when do moxibustion by yourself. It is better to help the aged people or kids to do moxibustion.
4. Ginger moxibustion group: Cut the fresh ginger into 0.2-0.4 cm thick, put it on the acupoints, and do moxibustion for 3-5 minutes on 3-5 acupoints, once a day.
5. Laser group: Helium and neon laser device, 3 mw output power, 2 mm spot diameter. Irradiate for 5 minutes on each acupoint, and choose 2-3 points every time.
6. Acupoints injection group: Three groups: ST7, Extra Tai Yang and GB14; Yifeng (TE17); ST2 and SI18; ST7, ST4 and Extra Qian Zheng. When the disease nearly heals and some local functions have not healed completely yet, apply one group of them or LI20, ST7, UB2 etc. to continue the treatment.
Injection dose of medicine and treatment course: Inject on one group with Securinine Nitrate of 0.4% concentration, 0.5 ml for each time. Lift the top of the needle when injecting, it is better to get the sore and distant sense. Local swelling appears during the treatment in a few patients; the swelling will dispears of itself after doses reducing or drug withdrawal.
Result analysis: The total effective rate is 98.82%. According to the Chinese medicine theory, the circulation of facial merdians are related with six yang meridians of hand and foot, three yin meridians of foot, conception vessel and govener vessel. In the early stage of facial paralysis, focus the treatment on six yang meridians to open the meridians, dispel the wind and remove the pathogens. The long term illness followed with facial spasm is related with three yin meridians of foot, and it is better to strengthen spleen and nourish muscles, tonify kidney and soften the liver. The curative effect will be improved if the prescription is based on the differentiation. Apply UB2 penetrating to GB14 for dysfunction of wrinkling forehead; apply UB2 penetrating to Extra Yu Yao for dysfunction of eyes closed; apply SI18 penetrating to ST2 for dysfunction of bloating cheeks; apply ST7 for dysfunction of showing the teeth; apply PC5, LV3, Sanyinjiao (SP6) and Taixi(KI3) Shu-Stream, Yuan-Source Point for facial spasm. The facial paralysis will cures soon if be treated with correct way.
