Stranguria

Internal Medicine

Type 1: Heat stranguria:
Symptoms:
Frequent micturition, difficulty in micturition, scanty in amount, yellow and turbid urine, burning sensation and stabbing pain in the urethra, the lower abdomen being lapse and full, or fever with chilliness, bitter taste in the mouth, dry stool, red tongue with yellow and greasy coating.
Principle of treatment:
Clearing away heat and eliminating dampness,promoting diuresis to relieve stranguria. Select the points from three Yin Meridianss, the Back-Shu and Front-Mu points. Puncture the points with reducing method.
Prescription:
Pangguangshu (BL28) (Pangguangshu (BL28)), Zhongji (CV3) Front-Mu Point of the Bladder (RN3), Yinlingquan (SP9) He-Sea Point (Yinlingquan (SP9) He-Sea Point), Xingjian (LR2) Ying-Spring Point (Xingjian (LR2) Ying-Spring Point), and Taixi(KI3) Shu-Stream, Yuan-Source Point (Taixi(KI3) Shu-Stream, Yuan-Source Point).
Modification according to symptoms:
For fever, Hegu (LI4) Yuan-Source Point (Hegu (LI4) Yuan-Source Point), Waiguan (TE5) Luo-Connecting Point the Eight Confluent Point (SJ5) are added. For calculi, Weiyang (BL39) Lower Confluent Point (Weiyang (BL39) Lower Confluent Point), Rangu (KI2) Ying-Spring Point (Rangu (KI2) Ying-Spring Point) are added. For hematuria, Zhaohai (KI6) The Eihgt Confluent Point (Zhaohai (KI6) The Eihgt Confluent Point), Sanyinjiao (SP6) (Sanyinjiao (SP6)) are added.
Type 2: Calculous stranguria
Symptoms:
Feeling bloated and stabbing pain in the lower abdomen and penis, sudden interrupted urination due to sand and stone, urination may continue by changing body position. Color of urine remains unchanged mostly. Yellow or bloody urine may appear when infection or collaterals stabbed by sand and stone. White or yellow and greasy fur, wiry and rapid pulse. If stone is located at the middle and upper part of the ureter, serious abdominal pain or lumbago may also occur, with pallid complexion, nausea, vomiting, and cold sweating, etc.
Principle of treatment:
Clearing away heat and promoting diuresis, relieving stranguria and removing the stones. Select the points mainly from three Yin Meridianss and Back-Shu points, Front-Mu points. Puncture with reducing method or combined reinforcing and reducing methods.
Prescription:
Pangguangshu (BL28), Zhongji (RN3), Yinglingquan (SP9), Xingjian (LR2), Taixi (KI3), Weiyang (BL39), and Rangu (KI2).
Modification according to symptoms:
For fever, Hegu (LI4), Waiguan (SJ5) are added. For hematuria, Xuehai (SP10) (Xuehai (SP10)), Sanyinjiao (SP6) are added.
Type 3: Hemorrhagic stranguria
Symptoms:
Frequent micturition, dripping and difficult micturition with stabbing pain, accompanied with streak of blood, and blood in urine. Little distending pain in the lower abdomen, yellow and greasy fur, or red tongue with thin coating, thready and rapid pulse.
Principle of treatment:
Cooling the blood and stopping bleeding, promoting diuresis and relieving stranguria. Select the points mainly from three Yin Meridians and Back-Shu points, Front-Mu points. Puncture with reducing method or combined reinforcing and reducing methods.
Prescription:
Pangguanshu (BL28), Zhongji (RN3), Yinlingquan (SP9), Xingjian (LR2),Taixi (KI3), Xuehai (SP10), Sanyinjiao (SP6)
Modification according to symptoms:
For fever, Hegu (LI4), Waiguan (SJ5) are added. For calculi, Weiyang (BL39), Rangu (KI2) are added.
Type 4: Qi-stranguria
Symptoms:
Distending pain in the lower abdomen and perineum, difficulty in discharging urine due to weakness, dysuria or even dripping in micturition; a desire to urinate frequently, deficient breath, soreness in the waist, listlessness, pale tongue, tiny and weak pulse.
Principle of treatment:
Supplementing the vital energy, promoting diuresis, and relieving stranguria. Select the point mainly from three Yin Meridianss, Back-Shu points and Front-Mu points. Reinforcing and reducing methods are both adopted. Moxibustion may be used if necessary.
Prescription:
Pangguangshu (BL28), Zhongji (RN3), Yinlingquan (SP9), Xingjian (LR2), Taixi (KI3), Qihai (CV6) (RN6), and Shuidao (Shuidao (ST28)).
Modification according to symptoms:
For turbid urine like chyluria, moxibustion may be added at Qihaishu (BL24) (Qihaishu (BL24)), Baihui (GV20) (DU20).
Type 5: Turbid stranguria
Symptoms:
Turbid urine like rice-wash with floating oil, and sediments that are like cotton, or accompanied by lumps or blood colour, steak of blood, and blood lumps. Difficulty in micturition. Dry mouth, white and slightly greasy coating, soft floating and rapid pulse.
Principle of treatment:
Benefiting the kidney and eliminatingturbid substance, promoting diuresis and relieving stranguria. Select the points mainly from three Yin Meridians, Back-Shu and Front-Mu points. Reinforcing and reducing methods are both adopted. Moxibustion may be used if required.
Prescription:
Pangguangshu (BL28), Zhongji (RN3), Yinlingquan (SP9), Xingjian (LR2), Taixi (KI3), Qihaishu (BL24), and Baihui (DU20).
Modification according to symptoms:
For hematuria, Xuehai (SP10), Sanyinjiao (SP6) are added. For weakness in micturition due to deficiency of Qi, apply moxibustion at Qihai (RN6), Shuidao (ST28).
